Ageing Well in Swansea

Dementia Friendly Swansea

We need your views

Ageing well is a priority for public services in Swansea.  Public services including Swansea Council, ABMU Health Board,  Mid & West Wales Fire and Rescue Service, South Wales Police and other local organisations are working together to ensure we are supporting the wellbeing of older people

An important part of this work is the Public Service Board pledge to work towards becoming a dementia friendly community. We want to know what we can do, to make Swansea a better place to live for people with dementia and memory problems.

The voices of people with dementia, people with memory problems and their carers are at the heart of the process of becoming more dementia-friendly and invite you to complete this attached questionnaire.  Alternatively you can complete the questionnaire online at:

www.swansea.gov.uk/ageingwellinswansea

www.abertawe.gov.uk/HeneiddionddaynAbertawe

Please return all completed questionnaires by 23rd February 2017

You can hand completed forms into your local library.

Or send it to: Polly Gordon, City & County of Swansea, room 3.3.16 Civic Centre, SA1 3SN

or email to polly.gordon@swansea.gov.uk
